Project Description
This program provides comprehensive primary health and dental care to residents
of Public Housing in Cleveland. It is focused at two PHPC health center sites,
one at the King-Kennedy Estates and one at Riverview Towers Estates. Services
are both physically and culturally accessible to the target population, and
strive to provide consistent, quality health and dental care to clients without
regard to their ability to pay, utilizing a sliding scale of fees.
Services
Oral health care, STD testing and treatment, prenatal and postpartum care,
well-child visits, immunizations, cancer screenings, gynecological
examinations, subspecialty referrals, health education, podiatry, focus on
diabetes through participating in a Diabetes Health Care Collaboratives and
health outreach.

Project Description
Columbus Neighborhood Health Center, Inc. (CNHC), was established in 1997
through a merger of five Section 330 (e) Consolidated Health Centers and a
Section 330 (h) Health Care for the Homeless program.
Services
A fulltime Medical Director administers a provide staff of physicians, nurse
practitioners and medical professionals offering: interpreter services; family
practice; internal medicine; OB/GYN; pediatrics, dentistry, optometry, health
and wellness; patient education; case management; referral services; podiatry;
smoking cessation; mental health services; nutrition services; homeless
services; enabling services; laboratory services through a contract with
Grant/Riverside (limited testing is done in-house); transportation and social
services are all available in the five, urban, state-of-the-art facilities
within the health care deliverly model. In addition, pharmacy services include: 340B under a contract with
Kroger Pharmacy; Sharing to Care; P.A. - Patient Assistance Program and a Tobacco Program.
