Project Description
The Hill Health Center PHPC program operates out of three Satellite clinics, which are located in New Haven, Ansonia, and West Haven. An outreach model is used which includes assigned satellite staff who provide health education, diabetes, and hypertension screenings on site. The program currently has three case manager/outreach workers who promote health fairs, facilitate medical, mental health, and substance abuse referrals to the Hill Health Center and to other community providers as required. On site Flu shots, some immunizations, asthma workshops, and assistance with obtaining medical insurance coverage is provided.

Services
The Hill Health Center offers services such as OBGYN, prenatal, immunizations, family planning, ultra sound, dental, mental health, child guidance, WIC, diabetes care, hearing screening, nutrition, pharmacy, smoking cessation, podiatry, parent education, health education, vision screening, head start, HIV, mammography, internal medicine, birth to three, detox and methadone, health promotions, nutrition, and weight promotion.

Project Description
Charter Oak Health Center, Inc. (COHC) is a federally qualified, freestanding, JCAHO-accredited, nonprofit health center that has continuously provided community-based ambulatory primary health care in Hartford since 1978. It was formed in response to the unmet health care needs of residents of two public housing projects. COHC is the only community health center located in southern Hartford. COHC provides an integrated model of primary care based on a private practice model through three major patient care departments: medical, dental, and behavioral health. COHC’s target population is primarily medically underserved and comprised of individuals and families who are members of racial/ethnic minorities and who live in poverty.

Services
COHC provides comprehensive primary care to all in need on a sliding fee basis without regard to ability to pay. In addition to its community health center sites, COHC operates several off-site programs in Hartford. Each of these sites has licensed space dedicated to primary health care and is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
