Project Description
West Side serves two public housing sites in St. Paul, MN that serve a predominantly refugee population (Hmong) and a growing Latino population. One clinic is located in the public housing community center and the other is community-based, approximately 2.5 miles from the public housing site. West Side provides comprehensive primary care services 5 days per week and offers extended hours urgent care services through the main clinic facility.

Services
Family Practice Care, midwifery services, bilingual/bicultural health education and outreach, diabetes case management services, social services, and Hmong Women's Support Groups for Depression and PTSD

The Boys and Girls Club, PHA - Space and Outreach Activities, ECFE - Education of Parent's Children, and Hmong Women's Health Coalition - Community baby showers and educational events
