Project Description
CommuniCare Health Centers acquired the Public Housing Primary Care project when it merged with the Ella Austin Health Center in 2000. Ella Austin Health Center was one of the 7 original PHPC grantees. CommuniCare's PHPC project serves approximately 82 public housing developments in San Antonio and Bexar County.

Services
Outreach - Address special needs of public housing and Section 8 residents with medical, dental, nutritional, and behavioral health services; Hospital and home visits

Health Education - Individual and group health classes; individual health checks (blood pressure, blood sugar, cholesterol); diabetes education, immunizations; STD prevention; family planning

Behavioral Health - Couple and individual counseling; problem solving; listening and communication skills; psychiatric consultation; support groups

Project Description
Centro de Salud Familiar La Fe provides comprehensive primary health care services to residents of public housing in El Paso County. Services are provided at a family housing site and a senior high rise.

Primary medical and dental services, immunizations, OB/GYN, family planning, free pregnancy and HIV testing, comprehensive HIV Care Center, laboratory, X-rays, pharmacy, health promotion, disease prevention, WIC, Senior Companion Program, CASASTART Program, Banco del Barrio (Time Dollar), CHIP Program, Child and Adolescent Wellness and Cultural and Technology Center.

Project Description
The Heart of Texas Community Health Center (HOTCHC) has a 37-year history of providing health care services in its community. HOTCHC’s mission is to provide high quality primary health care to increase access and eliminate disparities in the provision of primary health care services to the vulnerable of McLennan County, Texas. HOTCHC provides health services to three public housing sites located in Waco, TX.

Services
HOTCHC provides an array of primary health care services including oral health, mental health as well as a specialty care including (ob/gyn, pulmonary, diseases and critical care, psychiatry, orthodontics, etc.) and diagnostic (lab, x-ray, bone density, ultrasound, pulmonary function testing, etc.) services.

Project Description
Houston Area Community Services, Inc. (HACS) provides outpatient primary care, pharmacy services, behavioral health, and housing-related services to individuals and families within the City of Houston/Harris County.  The Joint Commission accredits our outpatient primary care and behavioral health programs.  The Texas Department of State Health Services licenses HACS’ intensive outpatient treatment (IOT) program for substance abuse.

HRSA designates HACS as a Community Healthcare Center (CHC) and Public Housing Primary Care (PHPC) provider.  The CHC component targets the Independence Heights and Spring Branch MUAs.  The PHPC section provides services to residents in surrounding public housing facilities.

Currently, HACS provides PHPC services to residents of Independent Heights and Jackson Hinds Gardens, both located in the Independence Heights MUA.  Our agency intends to expand to other public housing facilities located within the City of Houston.

Project Description
Community Healthcare Center (CHC)-Juarez Clinic will begin providing primary and preventive healthcare services to the residents of the three Wichita Falls public housing communities and the surrounding areas by mid-2009.  In collaboration with the Wichita Falls Housing Authority, services will be provided at a newly renovated facility within one of the public housing communities to ensure timely access to services.  Access to primary and preventive dental services will also be provided to these communities via CHC’s mobile dental van.

Services
CHC will offer primary and preventive medical and dental services.  Referrals will be coordinated with various local-area specialists as needed.  Services and essential clinical support to be offered at the Juarez Clinic include medical and dental diagnosis and treatment; dental hygiene; class-D pharmacy and lab; prevention services of adult physicals, children’s Texas Health Step exams, immunizations, cancer screenings (breast, cervical and prostate) and cancer prevention education (skin and lung); referrals to medical specialists; chronic disease Health education; financial and program eligibility screening; Medicaid assistance; case management; and translation.
